Health Risk Assessment
Identifying lifestyle risk factors
is the first step toward reducing medical claims and developing a
healthier, more productive workforce. A
Health Risk Assessment (HRA) is the
single-most reliable indicator of preventable healthcare claims.
On-Site Screenings
Biometric data is recorded on the
HRA. Professionally staffed on-site screenings are quick and comprehensive.
Lipid panel, Glucose, Blood Pressure, Height/Weight, & body composition are
collected. Additional information can be added for more detailed reporting.
Mobile Screenings
We have the capability to dispatch
professionally trained mobile collectors to accommodate busy executives or
telecommuters that cannot attend on-site screenings at your facility.
Personal Report
Each employee receives a health
status report based on their answers to the questionnaire as well as their
biometric data. It contains a wellness score as well as an action plan
to improve the score.
Aggregate Risk Reporting
After each screening event, an
Aggregate Risk Report is generated to quantify total risks within the
population as well as categorize each health risk. This report is beneficial in
targeting interventions for greatest economic impact.

Personal Health Coaching
Preventive vs. Reactive
My Wellness Matters Health Coaches are trained
specifically to address reducing health risks to prevent or delay onset of chronic diseases. This
approach differs from traditional “disease management” programs that work with
candidates to manage their lifestyle after claims have occurred.
The ultimate goal is to help
individuals prevent or reduce risk factors associated with heart disease,
diabetes and/or cancer. Specific areas of focus include weight management,
physical activity, nutrition and smoking cessation.
Outcomes Measurement &
Evaluation
We understand that data collection is an activity, not
an outcome. As the measured result is our focus, we analyze and evaluate the
outcome of the interventions utilized throughout the program for maximum ROI. A
sampling of the outcomes typically measured include:
